Vanderbilt University Medical Center (VUMC) in Nashville, Tennessee, announced on February 21, 2026 that it will cease providing gender affirming surgical procedures for adults due to "operational limitations and lack of surgical coverage."

The surgeries being discontinued include orchiectomies (testicular removal surgery) and subcutaneous mastectomies (removal of breast tissue, commonly known as "top surgery"). Previously scheduled surgeries will still take place, but VUMC is no longer scheduling new procedures or consultations.

Patients were informed of the change through their medical portals, which cited the departure of the senior surgeon responsible for gender affirming plastic surgeries at the facility.

Vanderbilt Health continues to provide nonsurgical gender affirming care for adults 19 years and older.

This statement from the hospital confirms that non surgical interventions, including hormone treatments, will remain available despite the surgical halt.

The decision marks a significant shift for the institution, which previously paused sex change surgeries for minors in 2022 following widespread public pressure. Deputy CEO Dr. C. Wright Pinson stated at that time that the facility was "pausing gender affirmation surgeries on patients under 18" to conduct a clinical review.

State bill SB2118, currently advancing through the legislature, would prohibit Medicaid (TennCare) coverage for sex change surgeries and puberty blockers for individuals diagnosed with gender dysphoria. In 2023, Tennessee enacted Senate Bill 1, which banned genital surgeries and puberty suppressing drugs for minors with gender dysphoria. The U.S. Supreme Court upheld this law in 2024 by a 6 to 3 margin, ruling that it does not violate Equal Protection guarantees under the Constitution.

Additionally, VUMC closed its LGBTQ health clinic in 2025, signaling a broader institutional pullback from these services.
